Street Outreach Programs Street Outreach

Funded by the City of Toronto, The Christie Ossington Neighbourhood Centre Street Outreach Team operates from Monday to Friday 11am till 7pm with longer hours during extreme cold months. Please read further for more details.

Our service area is North to St. Clair, East to Avenue Road, South to Queen and West to Jane.

WHO WE ARE

The Street Outreach Program is intended to bring homeless people out of the street and into appropriate shelter, with the intention of linking further with income, housing, medical help and life skills development and employment support. We provide informal counseling, information, referrals and transportation to homeless people. Neighbors and businesses are encouraged to call the CONC outreach team whenever a homeless person or a vulnerable community member may need support.

OUR PARTNERS

The Outreach Program maintains constant communication with all agencies providing High Support Street Outreach: * Albion Neighborhood Services (serving Etobicoke), * Community Mental Health Centre (serving North York) * NA-ME-RES (serving Mid-Town) * Salvation Army Gateway (serving East York) and Shelter Access Van (Shelter to Shelter service) * Agent court street outreach (serving scarborough)

OUR NEIGHBOURHOOD

The Street Outreach Program provides transportation, support, information and referrals to the community. The route for the outreach van includes the area between College street and Davenport Road; Roncesvalles avenue on the west and Avenue road on the east.

OUTREACH TEAM

Outreach workers are available with the outreach van for informal counseling, transportation information and referrals to vulnerable and homeless members of the community.
